Wie seine pharmakologischen Verwandten Terazosin und Doxazosin wird er vorrangig zur Behandlung der arteriellen Hypertonie eingesetzt. Prazosin wird auch bei Patienten, die an dem Raynaud-Syndrom (Durchblutungsstörungen in den Fingern) leiden, angewandt.
